APPOINTMENTS
 
famaily dental care Because we value each patient’s time and busy schedule, our staff strives to not keep our patients waiting. We encourage you to be on time – we don’t want to keep other patients waiting. We know that an unexpected event might keep you from keeping an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee. If you find that you cannot keep your appointed time, please be sure to call our office at your earliest convenience.

INSURANCE
 
Payment is expected at time of treatment. As a service to our patients, we will verify and file insurance for you. Our office is committed to helping you maximize your insurance benefits. It is important that you provide all insurance information to our office prior to beginning treatment. We can only estimate your portion and cannot guarantee coverage due to the complexities of insurance contracts. Any remaining balance after insurance payment is received is the patient’s responsibility and will be due within 30 days. We accept Cash, Checks, MasterCard, Visa, Discover and CareCredit.

We have included our Patient Acquaintance, Financial Responsibility and Assignment of Insurance Form as well as our Medical History Form. If you would like to download, complete and bring these with you to your first appointment. This will save you time.
